No Scalpel Vasectomy

Vasectomy overview

Vasectomy is dependable method of  birth control for men who think they will never want any or any more children. It was developed in early 20th century and gained popularity in 1950s. It is the name of a surgical procedure which makes men sterile and is the most efficient contraceptive method.

Many Australian men are turning to vasectomies for an effective and safe method for family planning. In fact, around one in four men over 40 have had a vasectomy procedure.

It is simple 15-20 minute procedure performed in doctor’s office or clinic under local anaesthetics. The aim is to prevent sperm from entering the semen during ejaculation, 95% of which is the support fluid from prostate gland and seminal vesicles located behind bladder.

The procedure involves cutting or blocking the tubes called vas deferens, which are the tubes that carry sperm from the testicles to the outside world.

It is important to note that while a vasectomy is considered a permanent form of contraception, it will take 3 months to obtain sterility.  You will be required to do a semen analysis in 3 months to obtain clearance for sterility. It is crucial to use alternative birth control methods until the absence of sperm is confirmed by your doctor.

After the surgery, patients may experience some mild discomfort, swelling, or bruising, but these symptoms generally subside within a few days. The mild discomfort is usually settled with Paracetamol and Ibuprofen. You can be back at work within 1-2 days.

Vasectomies are now-routine procedures for men who no longer want more children or couples that don’t want children.

No-scalpel, fascial interposition, open-ended vasectomy

Dr Modi perform simple, quick, minimally invasive vasectomy procedures with minimal discomfort.

We snip two tubes. These tubes are called vas deferens.

These tubes are the highway your sperm takes on it’s way to the outside world. We seal off one end of the tubes & bury them over two different layers in the scrotum. The procedure is then complete.

Sperm makes up only 5% of ejaculate fluid. Therefore, you will continue to produce ejaculate fluid. Just no sperm will make its way out with no impact on your sexual function.

Anaesthesia for vasectomy

A local anaesthetic agent is injected using a  tiny needle  to numb the scrotal skin and vas deferens.

This provides pain relief for up to 12 hours and allows you to drive to and from our clinic yourself.

Limitations of Vasectomy

  •  It doesnt protect one from trasmission of  sexually transmitted diseases.
  • Although reversal has good success rate, but its not 100%
  • Must use other methods of contraception until sperm-free which is about 3 months or about 20 ejaculates.

About the procedure

Anaesthesia: An anaesthetic solution lignocaine is injected with a tiny needle to numb the scrotal skin and the vas tubes.

Access:  In conventional vasectomy, one or two 1/2-inch scrotal incisions are made
to gain access to the vas tubes. These incisions are later closed with sutures,
which remain in place for about 5 days until they dissolve or are removed.

During no-scalpel vasectomy (NSV),  with help of special instruments,  a single tiny access-opening on the front side of the scrotum is made. This small slit of about 1 cm  usually seals within hours, so no stitches are needed.

Blocking the Vas tubes:  Each vas (left and right)is divided under direct vision with fine surgical scissors about an inch above each testes. Nothing is removed; the ends of the divided vas are placed out of alignment and by applying tiny clip to the sheath (fascial interposition), it  puts one end inside the sheath and the other outside.  By doing so, the divided vas is placed out of alignment and kept from rejoining . The While extremely effective ( failure rate less than 1 in 2000), the technique provides for easier reversal in men who choose reversal later in life.

It is important to note that while a vasectomy is considered a permanent form of contraception, it will take 3 months to obtain sterility. You will be required to do a semen analysis in 3 months to obtain clearance for sterility. It is crucial to use alternative birth control methods until the absence of sperm is confirmed by our staff.

After Vasectomy

Recovery from vasectomy is mostly uneventful but may  involve some discomfort, swelling, and possible bruising. It’s important to rest and follow all post-operative care instructions provided by your doctor. Engaging in light activities and avoiding heavy lifting is generally recommended during the initial recovery period.

There are still live sperms in the portions of the vas tubes within the pelvis until they are all released. It usually takes about 20 ejaculations and 12 weeks to become sperm free. In 5% of men, the semen may still contain some sperm for months (usually only few in number and not active), so it is very crucial to have semen sample checked at 12 weeks after vasectomy. Meanwhile, you’ll have to use other forms of contraception until the semen is checked by pathology lab and reported as semen is sperm-free.

Sperm are still produced by the testes but can no longer pass through the tubes. What happens is that now body’s white blood cells ingest and disintegrate the retained sperms and its proteins are recycled back into the system.

There is   No change in erections,  No change in semen,  No change in libido

What to expect

Pre and Post-Op Care

WHAT TO EXPECT

Pre and Post-Op Care

  • Pre-op prep

  • Post-op care

  • 3 month sperm test

Pre-op prep

  • Shave off all pubic hair on the scrotum. This minimises the risk of infection.
  • Wear tight underwear. This is to minimise post-operative swelling.
  • Do not take any anti-inflammatories or fish oil within 7 days pre-vasectomy. This includes Aspirin, Diclofenac and Ibuprofen. These drugs thin out the blood and can make the procedure more difficult.

Post-op care

  • You must have complete rest on the day of the surgery.
  • You can shower 24 hours after the procedure but keep the wound as dry as possible.
  • You will need to have a sperm test in 3 months. Before then, you must avoid unprotected sexual intercourse.
  • After your procedure you may notice light spotting on the first day and some swelling and discomfort for a week or so. Any pain or discomfort you experience during your vasectomy recovery can be treated with pain killers. Take Paracetamol for pain relief. Only use ibuprofen after 1 week if necessary. Apply cold packs to the scrotum the first few days after your vasectomy.
  • It is important you wear tight underwear in the first few days after your vasectomy.
  • Depending on the type of work you do, you can return to work two days after the procedure. This is the case for office workers. If your job involves reaching, stretching or any other physical exertion you may request to be put on light duties for one week. If this is not possible, you will need to take the week off. We can provide you with a medical certificate.
  • Avoid contact sports, running, cycling or heavy lifting for 4 weeks.
  • Abstain from sex and masturbation for the first week after the vasectomy. Abstain from vigorous sex for 2 weeks.
  • Once you are physically able to have sex use other forms of contraception such as condoms until your 3 months ejaculate test proves you to be infertile. Once proven to be infertile you may engage in unprotected sex.
  • It’s important to remember that your vasectomy won’t protect you from any sexually transmitted infections (STI). In situations where there may be a risk of acquiring an STI please use a condom.
  • Your vasectomy will not impact your sexual performance or ability to ejaculate.

3 month sperm test

  • Post a vasectomy you are not considered sterile until you complete your semen analysis.
  • This must be completed 3 months post a vasectomy.
  • On the day of your vasectomy you will be given a referral and instructions on how to have your semen analysed.
  • Once you have done this, we will receive a report from the pathology lab.
  • We will then contact you and ask you to book a quick phone consult with our doctor to discuss your results.
  • You are deemed sterile only after our doctors have cleared you.
